The Health Insurance Portability and Accountability Act of 1996 (HIPAA) mandated that the Secretary of Health and Human Services adopt a standard unique health identifier for health care providers. On January 23, 2004, the Secretary published a Final Rule that adopted the National Provider Identifier (NPI) as this identifier.

All HIPAA covered healthcare providers, whether they are individuals or organizations, must obtain an NPI for use to identify themselves in HIPAA standard transactions. Once enumerated, a provider's NPI will not change. The NPI remains with the provider regardless of job or location changes.

HIPAA covered entities such as providers completing electronic transactions, healthcare clearinghouses, and large health plans, must use only the NPI to identify covered healthcare providers in standard transactions by May 23, 2007. Small health plans must use only the NPI by May 23, 2008.

Below you will find additional information and resources as to how to apply to obtain your NPI number.

How to Apply

Individual providers should apply now for an NPI. Health care providers who participate in a group practice, or are organizational providers, should coordinate with their organization to determine the appropriate time for NPI application.

Health care providers can apply for NPIs in one of three ways:

  1. By using the web-based process (National Plan and Provider Enumeration System (NPPES)). This process was available beginning May 23, 2005.

  2. By filling out a paper NPI Application/Update form and mailing it to the Enumerator (NPI Application/Update Form [PDF, 68KB]). This process was available beginning July 1, 2005.

  3. By having their NPI application information, along with the application information for many other health care providers, submitted to NPPES on their behalf in an electronic file by an organization. This process is known as "electronic file interchange" (EFI) for bulk enumeration. Providers must give their permission for an organization to do this. This process is not yet available. For more detailed information on EFI, please refer to the EFI web section.
