our site and how we use it. St. Joseph Heritage Healthcare reserves the right to

Joseph Heritage Healthcare Network Support Services (SJHH NSS) you unconditionally
agree to be bound by this Privacy Policy.
Information Collection and Use:
SJHH NSS collects and processes information about you in order to:
(i) identify you each time you visit a Web Site or wish to have a Service provided;
(ii) process orders or applications submitted by you;
(iii) improve our Services and Web Sites; and
(iv) allow access to our Service.
In addition, SJHH NSS allows users to apply for an account to access claim information
that will contain Protected Health Information (PHI). At different places on our
site, you may be asked to volunteer your name, address, provider's tax identification
number, and telephone number. We refer to this type of information as Personally
Identifiable Information (PII) because it can be used to identify or contact you.
We understand that the PII and Personally Identifiable Health Information (PIHI)
you provide during your visit to our site is highly sensitive and you may have concerns

Log files/IP addresses:

by our web server. We use IP addresses to help us administer our Web Site and to

in providing an effective service on our Web Sites. We may from time to time supply

our Web Sites with information relating to the number of users linking to our Web
Sites from their sites. You cannot be identified from this information.

warrant, law or regulation. In the event that we are compelled to disclose personal
information and/or health-related personal information to a third party, we will
notify you unless doing so would violate the law or court order. We also reserve
the right to conduct an internal investigation or in limited cases cooperate with
law enforcement authorities in investigating and prosecuting users that violate
our rules or engage in behavior that is harmful (or illegal) to other users. Your
personally identifiable information will not be distributed or sold to third-party
organizations.
